what needed to change

remote care can create visibility gaps when vitals are monitored inconsistently, data is fragmented, or clinicians lack a clear way to identify what needs attention.

our client needed a more proactive monitoring experience that could support earlier risk detection and more consistent decision-making across care settings.

at the same time, the platform had to work for different users, from patients tracking their own vitals to clinicians monitoring multiple patients and responding to emerging risks, while keeping complex technology intuitive enough to encourage adoption without adding friction to demanding clinical workflows.

how we designed for It

research, design, and validation moved together around one goal: make critical information easier to see, understand, and act on.

design around clinical decisions

we mapped the patient-to-clinician journey to understand where information was needed, how risk was assessed, and where delays could affect care. the resulting experience brought relevant patient data and risk signals closer to the decisions clinicians needed to make.

connect patients and clinicians

we designed complementary experiences across mobile and desktop, allowing patients to capture and share vitals while giving clinicians a real-time view of patient status. this created a connected workflow rather than separate monitoring tools.

turn signals into action

risk scores, alerts, and clinical recommendations were built into the experience to help clinicians identify meaningful changes faster. instead of overwhelming users with data, the platform surfaced information in ways that supported timely intervention.
